Implementing a policy of routine opt-out HIV testing led to the diagnosis of 3000 HIV infections in children admitted to hospital in Lusaka over an 18-month period, investigators report in a study published in the online edition of the Journal of Acquired Immune Deficiency Syndromes. “Increased HIV testing has been proposed as an important component of HIV prevention and a pathway to support universal access to antiretroviral therapy”, write the investigators. “This is particularly important for infants where the rate of disease progression is extremely rapid, the risk of early death is high, and antiretroviral therapy can decrease mortality significantly in the first year of life”, they add. In an attempt to increase the number of diagnosed HIV-positive children in Lusaka, Zambia, investigators at the University Teaching Hospital implemented a policy of offering routine HIV counselling and testing to all children admitted as inpatients. HIV prevalence amongst women attending antenatal services in Zambia is 25% and an estimated 28,000 infants are born each year in the country with HIV infection. The investigators therefore hypothesised that routinely offering HIV testing would identify large numbers of HIV-infected children. A total of 15,670 children of unknown HIV status were admitted to the hospital as in-patients between January 2006 and June 2007. Of these, 13,239 parents/caregivers (85%) received counselling for testing of their child and 11,571 children (87%) had an HIV test.

Objectives: To develop, implement and evaluate the processes of a novel approach to chlamydia testing that is accessible, confidential, free of charge, easy to use, and allows for self-collection of specimens, their transportation by regular mail and the central management of results. Methods: A 'self-collection kit' was developed including all items and instructions necessary to obtain a sample. A network of distribution sites at locations frequented by the target population has been established. The 'kits' can be requested via an advertised website and by phone. Specimens are returned via reply paid mail. A centralised system for the management of results and follow up has been established. Test results are conveyed by the participant's method of choice. Treatment is organised via a network of health care providers. Results: Of the first 100 returned kits 99% were safely packed and 86% were sent back with a completely filled out pathology request form. Ninety-two participants provided contact details; 70.7% indicated mobile phone or SMS as the preferred methods to receive results. Seven positive cases were identified and treatment was confirmed for all within 6 days. Discussion: These findings provide evidence that the presented approach to chlamydia testing is easy to implement, achieves excellent follow-up and treatment rates, and therefore opens important new channels to otherwise difficult to access high-risk populations, such as young people and geographically and socially isolated populations.
 
Can the Swedish new variant of Chlamydia trachomatis (nvCT) be detected by UK NEQAS participants from seventeen European countries and five additional countries/regions in 2009?

In 2006, a new variant of Chlamydia trachomatis (nvCT) was reported in Sweden. The nvCT contains a deletion that includes the targets for the C. trachomatis genetic diagnostic single-target systems from Roche Diagnostics and Abbott Laboratories. Roche and Abbott have now developed certified dual-target assays that can detect the nvCT. This study examined the nucleic acid amplification tests (NAATs) currently used (in 2009) for C. trachomatis detection in laboratories from 17 European countries and five countries/regions outside Europe that are participating in the United Kingdom (UK) National External Quality Assessment Service (NEQAS). It further examined changes in these laboratories' testing strategy during the period from 2006 to 2009, and their performance regarding nvCT detection. A UK NEQAS blinded nvCT specimen was distributed to all 283 participating laboratories, which were asked to analyse the specimen according to their routine C. trachomatis diagnostic protocols for endocervical swabs. BD ProbeTec was the most commonly used NAAT, followed by Cobas Amplicor, Cobas TaqMan, and Aptima. From 2006 to 2009, the use of Cobas Amplicor, which does not detect the nvCT, decreased, but it was still used by 22% (n=57) of responding participants in 59% of the countries, 54 of these 57 used it as first assay. Virtually all of the other participants detected the nvCT correctly. Laboratories using commercial or in house NAATs that do not detect the nvCT are encouraged to carefully monitor their C. trachomatis incidence, participate in effective internal and external quality assurance and controls schemes, and to consider changing their testing system.
